Peripheral Exudative Hemorrhagic Chorioretinopathy (PEHCR): Diagnostic and Therapeutic Challenges

Background and Objectives: Peripheral exudative hemorrhagic chorioretinopathy (PEHCR) is a peripheral retinal vascular abnormality that likely underreported. We review the differential diagnoses, etiology, treatment options for PEHCR. Methods: present case of an asymptomatic 72-year-old female referred following left eye fundus photography finding lesion. Results: Fundus demonstrated large temporal pigment epithelial detachment (PED) with adjacent fibrovascular membrane. Optical coherence tomography (OCT) confirmed PED trace subretinal fluid. Fluorescein angiography (FA) early late hypofluorescence leakage Observation was elected, visual acuity remained unaffected, spontaneously resolved. Conclusions: Due to location, patients often as asymptomatic; however, vision loss can occur due vitreous hemorrhage or extension fluid, hemorrhage, exudate macula. Commonly, these lesions are concern choroidal melanoma their large, dark, elevated presentation in retina. Multimodal testing using B-scan, FA, OCT important establishing proper diagnosis. PEHCR be observed without treatment, though intravitreal injection anti-VEGF increasingly used prevent secondary causes loss.
